This scene was really the closure of the earlier section "On Mircandalee and Kerry", where Kes outlines her doubts and fears now Vizh has twins to occupy his affection. It's every older child's secret worry, but of course Kerry being Kerry it can become an explosive paranoia.

At the end of that scene we hear:

“Me and Vizh, talk?” Kerry snorted. “We only ever really connect when one of us has screwed up to epic levels or the world’s about to end. Or both.”

“Be careful what you wish for,” Mircandalee said superstitiously.

This is the chapter where that bit of foreshadowing comes true.

How would anybody know this?
